Fees
If you pay at the time of service or fees are as follows:
1/2 hour $35
1 hour $65
90 minute $95
2+ hour $65/hour, available in Thai style only.
Insurance Billing:
We bill insurance companies $25/15 minute increment or $100/ hour.
Motor vehicle accidents/ PIP claims--bring a prescription.
Worker's Compensation claims.
Health Insurance:
- Regents Blue Cross/ Blue shield of Oregon: all therapists are preferred providers.
- Lifewise of Oregon: Sarah Carl, Lindsay Wells, preferred providers.
- CHP: The holistic health care plan from Kaiser. All therapists are providers.
We have also worked with other common insurance companies. For more information about your massage benefits, call or e-mail our office. Be ready with your insurance card or your name, date of birth, policy and group numbers and any other information that could help us research your benefits.
